Studied the paper forms
Collected every paper form in use and turned the fields into a structured schema — no free-text catch-all dumping grounds.
Built mobile-first entry
Livewire + Alpine for fast, low-JS forms that work on a shift supervisor's phone in a parking lot with one bar of signal.
Added photo uploads and geotags
Direct-to-S3 uploads with thumbnails, plus optional GPS on submission. Evidence attaches to the event, not a separate folder.
Wired a full audit trail
Every field change, every status transition, every attachment — logged with actor and timestamp. Nothing disappears.
